After verifying that Pastor Flint is available for your ceremony, you may return your ceremony planning information in one of two ways.  One way is to print this page and then complete the blanks and mail it in.  If you have changes to the ceremonies, you can print them and then write your changes on them.  If there are any questions in preparing the ceremony, we will ask you for clarifications, and even show you a copy of the notes for your review if you would like.

Another way (which is much easier if you don't have significant changes or additions) is to complete the ceremony planning online page and submit your information there.
